How to Prevent Accidents in Confined Spaces

✅ 1. Identify and Assess Confined Spaces

  • Recognize confined spaces: tanks, silos, manholes, pipelines, etc.

  • Conduct a risk assessment to identify potential hazards:

    • Toxic gases or vapors

    • Flammable or explosive atmospheres

    • Lack of oxygen

    • Engulfment hazards (eg, liquids, sand, grain)

    • Mechanical or electrical hazards

Use 2. Use a Permit-to-Work System

  • Require a Confined Space Entry Permit before entry.

  • Make sure the permit includes:

    • Hazard identification

    • Isolation of energy sources

    • Emergency procedures

    • Entry time and duration

. 3. Air Testing and Monitoring

  • Test the atmosphere before and during entry:

    • Oxygen levels (safe: 19.5% - 23.5%)

    • Toxic gases (eg, hydrogen sulfide, carbon monoxide)

    • Flammable gases

  • Use calibrated gas detectors and continuously monitor air quality.

✅ 4. Ventilation

  • Provide adequate mechanical or natural ventilation.

  • Use exhaust fans or blowers to remove harmful gases and supply fresh air.

  • Keep ventilating during the entire operation.

✅ 5. Isolate the Space

  • Lockout / tagout (LOTO) of all energy sources: electrical, mechanical, hydraulic, pneumatic.

  • Block or disconnect pipes, valves, and moving parts.

✅ 6. Provide Proper Personal Protective Equipment (PPE)

  • Depending on the hazards:

    • Respirators or self-contained breathing apparatus (SCBA)

    • Protective clothing

    • Hard hats, gloves, and eye protection

  • Ensure workers are trained in the correct use of PPE.

. 7. Ensure Communication and Supervision

  • Maintain constant communication between entrants and a safety attendant (standby person).

  • Use radios, visual signals, or hardwire systems.

Train 8. Train Workers and Rescuers

  • Workers must be trained in:

    • Hazards of confined spaces

    • Safe entry and exit procedures

    • Use of equipment and PPE

  • Rescue teams should be trained and available on standby.

Have 9. Have an Emergency Rescue Plan

  • Create a site-specific rescue plan before entry.

  • Include procedures for:

    • Non-entry and entry rescues

    • First aid and medical response

  • Never rely solely on emergency services; have trained in-house responders.

✅ 10. Follow Regulations

  • Comply with local regulations:

    • OSHA 29 CFR 1910.146 (US)

    • HSE Confined Spaces Regulations 1997 (UK)

  • Keep documentation and training records up to date.
